?? Contract: 12-week temporary placement

?? Hours: Monday to Friday, 9am - 5pm

This role offers the opportunity to make a real difference by conducting adult social care assessments and support planning, working within NHS community social care offices in Torquay and Paignton.

Key responsibilities include:

• Conducting adult social care assessments and support planning (S.9)
• Mental capacity and safeguarding assessments (S.42)
• Risk assessments and carers assessments

We'd love to hear from professionals with a strong knowledge of the Care Act 2014 and Mental Capacity Act 2005 who can work independently and collaboratively in a multi-disciplinary team. Join us to help improve hospital discharge planning and commissioning services.

Strong knowledge of the Care Act 2014 & Mental Capacity Act 2005, ability to work independently and collaboratively, experience in hospital discharge planning, and excellent communication, time management, and IT skills are essential.
