Key Responsibilities:

• Delivering care plans for patients and ability to work efficiently within a busy general ward/department
• Monitoring vital signs
• Observing patient’s condition and recording information
• Communicating with patients and their families
• Supporting with personal care, comfort and dignity
• Maintaining accurate care records and reporting concerns to senior staff
• Working collaboratively with the day & night team to ensure a safe, calm environment
• working in a nursing home supporting residents with dementia, palliative care, stroke & complex care needs

Essential Qualifications & Experience:

• Care Certificate, Health & Social Care Certificate or equivalent
• At least 6 month’s experience working in NHS hospital, nursing home, care home & hospice supporting residents with dementia, palliative care, stroke & complex care needs within the UK.
• A caring, reliable and patient approach
• Excellent communication skills, both verbal and written, with the ability to build rapport with patients.
• Ability to work effectively within a team while also demonstrating initiative in patient care.
• Compassionate approach with strong interpersonal skills, demonstrating empathy towards patients' experiences.
• Proficiency in IT systems for maintaining patient records and accessing medical information.
• right to work in UK
• Enhanced DBS clearance required.
