Assists in the examination and treatment of patients under direction of physician or advanced practice provider by performing patient assessment procedures including but not limited to vital signs, obtaining/monitoring IV lines and infection disease control. Helps facilitate the flow of patients through the department.
Managing patient care is a team effort that involves clinical and nonclinical staff interacting with patients and working to achieve a successful patient centered medical home. Daily team huddles and regular staff meetings are a very import part in maintaining the care team.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Responsible for skillfully obtaining IV access, monitoring IV lines every 10 minutes and correct inefficiencies/obstructions in the IV tubing. If needed will find a new venous site.
Able to recognize critical symptoms and reports them to the physician/advanced practice provider immediately.
Initiates and assists in emergency procedures/therapeutic measures.
Interviews patients, measures vital signs, such as pulse rate, temperature, blood pressure, weight, and height, and records information on patients' charts.
Prepares treatment rooms for examination of patients. Serves as a patient chaperone per policy.
Explains procedures and treatments to patient to gain cooperation, understanding, and ease apprehension. Prepares equipment and assists physician/provider during treatment and examination of patient.
Administers prescribed medications, injections and treatments in accordance with policies/procedures. Observes patient, records significant conditions and reactions, and notifies supervisor or physician/provider of patient's condition and reaction to drugs, treatments, and significant incidents.
Works closely with providers and other support staff to assure patients are triaged based on symptoms and severity. Assess the condition/problem, asks pertinent medical history, and gives medical advice based on protocols set by providers. Consults with provider when appropriate. Makes decisions for further evaluation or follow-up as needed. Schedules work-in appointments if indicated.
Operates computer to process, store, and retrieve health information. Maintains accurate, complete, and timely patient records
Accesses Phytel to assist with pre-visit planning daily.
Supports PCMH (Patient Centered Medical Home Concept) by adhering to standards, policies, and procedures.
Provides patient education. Actively involved with encouraging patients and educated patient on lifestyle changes as relates to disease processes.
Communicates daily with clinical supervisor/administrative supervisor to identify areas of needed support for scheduling, pre-visit planning, and other PCMH activities.
Daily team huddles and pre-visit planning include but are not limited to:
- Reviewing appointments for the day and making sure all reports/x-rays/follow up correspondence/missing immunizations, etc. are complete and in the medical record for those patients.
- Discussing clinical concerns/information with appropriate provider prior to appointment.
- Reviewing charts for identified patients with important conditions (per PCMH and practice standards).
- Completing order inquiries to assure all orders have been documented
· Discuss what is going on in the practice that day that might affect patient care/flow.
Schedules tests and procedures for ancillary services. Obtains appointments with referring physicians and initiates the referral process.
Assesses and dresses wounds. Removes sutures and/or ADA staples.
Facilitates communication between patient, physician, and family as appropriate. Instructs the patient and family how to administer injections and the use of glucose monitors.
Accurately collect and process specimens for testing. Runs quality control tests on waived kit tests/machines. Perform waived tests and accurately document results in Centricity.
Responsible for handling by phone or mail questions and requests from patients regarding their medical care.
a) Promptly receives and responds to incoming phone calls/mail
b) Answers questions and requests regarding follow-up care, prescriptions, laboratory results, etc.
Complete prior authorizations for medications and patient referrals done in a timely manner.
Accurately completes forms (disability and FMLA) in a timely manner.
Maintains knowledge of and complies with established policies and procedures.
Responsible for maintaining patient confidentiality.
Other duties may be assigned.
May be required to travel to satellite locations.
