Salary: £23 - £29/hour

ROLE REQUIREMENTS:

Working in integrated/partnership within the Community Mental Health Team/East London NHS Trust.

Provision of a social work service within the framework of the Care Programme Approach & Care Act plus other appropriate legislative requirements.

Comply with the statutory requirements of the relevant childcare, mental health or Care Act legislation and in line with the requirements of the Council's policies and procedures.

Respond to requests for assistance, taking sufficient basic information about the needs in question to be able to determine the level and type of assessment required as per Care Act and other legislative requirements.

Ensure that care plans include clear statements of agreed outcomes. Service or resource to be provided via the care plan and in line with Council requirements.

Assist users and carers to play a full part in care planning by extending to them advice, support and guidance, including advocacy, and arranging where necessary access to translation, interpretation services.

Review the progress of each care plan at regular, pre-determined intervals (within one year minimum) together with users, carers, advocates and other service providers to ensure the effectiveness of the plan to meet agreed outcomes.

Have effective liaison and working relationships with mental health agencies and service providers and the wider council and community.

Attend case conferences, case meetings, police and/or court requirements and team meetings as required.

Carry out responsibilities under mental health legislation and regulations including, where applicable, Approved Mental Health Professional duties.

Develop and implement plans aimed at resettling people with severe mental illness into the community.

Participate in office duty arrangements and on-call arrangements.

Prepare and present reports on individuals (e.g. at Mental Health Review Tribunals, Court). Mental Capacity Act requirements etc

Approach

* Mental Capacity Act and other legislation which impacts on social work practice. Understanding of antidiscriminatory practice and the needs of diverse communities

Qualifications & Experience

* CQSW, DipSW or other CCETSW recognised social work qualification.

* Approved Mental Health Professional or will undertake requisite training.

* Substantial postqualifying experience.

* Substantial Mental Health Experience (including work with mentally ill clients and their carers)

* Working in a multiracial, multi-disciplinary and multi-agency area

* Practice teaching experience and/or interest in supervising social work and AMHP students

* IT literate and willing to undertake further IT training as required
