Job description

To provide high-quality, person-centred support to young people aged 16-25 who are homeless or at risk of homelessness. This role involves working with young people to build the skills, confidence, and resilience they need to move towards independent living and a more stable future.

Key Responsibilities:

* Build positive, trusting relationships with young people, offering emotional support and practical advice.

* Conduct assessments to identify individual needs, risks, and aspirations.

* Develop and regularly review support plans tailored to each young person's goals.

* Support young people with housing, education, employment, training, health, and life skills.

* Liaise with external agencies (e.g. housing providers, social services, mental health teams) to advocate for and coordinate services.

* Assist with tenancy sustainment, helping young people understand their rights and responsibilities.

* Respond appropriately to safeguarding concerns and follow organizational safeguarding procedures.

* Maintain accurate and up-to-date records using case management systems.

* Encourage participation in activities that promote well-being, inclusion, and independence.

* Contribute to a safe and supportive environment within the accommodation or service setting.

Person Specification:

Essential:

* Experience working with young people or vulnerable individuals.

* Knowledge of issues related to youth homelessness, mental health, and substance misuse.

* Strong communication and interpersonal skills.

* Ability to work independently and as part of a team.

* Understanding of safeguarding principles and confidentiality.

* Flexibility to work shifts, including evenings or weekends if required.

* Basic IT skills for record-keeping and communication.

Desirable:

* NVQ 2 Health and Social Care

* Experience working in supported accommodation or Homeless outreach settings.

* Naloxone Training
