About this Registered Nurse (RN) Evening Supervisor Position

HopeHealth is seeking an experienced RN for its affiliated HopeHealth Hospice & Palliative Care (HHHPC).

The evening supervisor is skilled professional providing nursing support ,teaching and training to the staff, patient, family, and/or caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of HopeHealth, and under the direction of the Clinical Manager for the Inpatient Unit.

The successful candidate is passionate about providing the best possible care at end-of-life and acts with a sense of urgency and a disciplined approach for the accuracy and quality of the care delivered.

This is a 40 hr full time position 3:00pm-11:30pm that is eligible for benefits.

What will you do as a RN evening supervisor?

  • Acts as a resource to the RN for all aspects of patient care
  • May assumes Charge RN responsibilities for the hours of 3PM to 11:30PM, taking referrals from the field staff, referral center and triage
  • Assists with the admission process as needed and ensures efficient, timely and safe patient transitions to and from the IPU.
  • Proficiency with administration of Omnicell functions
  • Assist staff with all bedside clinical applications including but not limited to trach care, chest drains, feeding tubes, wound care, starting and accessing peripheral lines, accessing an ports, dressing changes
  • Initiates and sustains as needed the implementation of orders for medications and treatments as prescribed by the physician; report / participate in investigations related to medication errors
  • Provides professional nursing care by utilizing all elements of nursing process. Coordinates all aspects of patient care from initial assessment through discharge. Assesses and evaluates patient’s status by:
    1. Developing and initiating plan of care in collaboration with patient/caregiver/primary health professional
    2. Regularly re-evaluating patient/family/caregiver needs and responding to changes as needed
    3. Conducting on-going evaluation and revision of care according to patient outcomes
  • Works collaboratively with other interdisciplinary team members.
  • Assesses patient environment and teaches patient/family/caregiver regarding home environment/safety issues when discharge to home is appropriate
  • Identifies and supervises ancillary services needed by the patient. Uses interdisciplinary approach in developing, supervising and coordinating plan of care, in accordance with the organization’s policies
  • Initiates referrals to other disciplines and community resources, as indicated and is necessary
  • Utilizes nursing assessment in establishing the certified nursing assistant care plan
  • Supervises LPN and paraprofessional staff
  • Assess Clinical Competencies
  • Assist Referral Center and Triage
  • Provide teaching and assisting on clinical topics and MyUnity functionality

What are the qualifications to be a Registered Nurse (RN) Evening Supervisor?

  • BSN or 1 – 3 years of related experience or training or related equivalent combination of education experience
  • Hospital care and/or hospice experience preferred
  • Demonstrated acceptable patient care and “high tech” skills
