What you will do:

• Conduct comprehensive mental health assessments for older adults within the community

• Develop and implement personalised care plans in collaboration with multidisciplinary teams

• Monitor and review ongoing progress, adjusting interventions as required

• Maintain accurate records and documentation in line with organisational and legal requirements
