Leaving Care Worker

A Local Authority is seeking a Personal Advisor to support young people leaving care and transitioning into independent adulthood.

Key Responsibilities:

Act as a named advisor for care leavers aged 18+
Support young people aged 16-18 alongside Social Workers to prepare for independence
Deliver one-to-one and group interventions to develop life skills and improve education and employment outcomes
Complete Needs Assessments and Pathway Plans
Provide advice on housing, health, education, and employment
Work with multi-agency partners to coordinate support
Undertake visits, maintain accurate records, and manage caseloads

Requirements:

Level 3 qualification in a relevant field or substantial equivalent experience
Experience working with vulnerable young people or care leavers
Knowledge of safeguarding and relevant legislation
Strong communication and organisational skills
Ability to work independently and manage competing priorities

Additional:

Flexible working, including occasional evenings and weekends
Ability to travel as part of the role
