Community Matrons focus on patients requiring short term medical and clinical intervention to treat and stabilize long term conditions, meet the holistic needs of the patient and ensure everything in place to optimise disease control. Input is short term and intensive to prevent admission to hospital and reduce the use of other services across the health and social care sector. As Southwark and Lambeth implement care coordination for patients with multiple long term conditions and move to a neighbourhood way of working, community matrons will work to deliver a population health approach to care working in a network with other specialists and services locally around the patient.

Our Community Matrons work alongside and in support of our Neighbourhood Nursing Teams, consisting of Nurses, Apprentice Nursing Associates and Nursing Assistants.

We are seeking an enthusiastic experienced Community Matron to undertake a dual focused role incorporating the management and leadership of our team of community matrons across Lambeth and Southwark, and carrying a clinical caseload.. We have a team of 8 community matrons and Deputy Matrons.

We require an experienced community matron, expert in advanced assessment, long term conditions and a registered Independent Prescriber

You will have responsibility for the day to day operational management and clinical leadership of the area of responsibility alongside a patient caseload. You will be developing the service and working up performance indicators to demonstrate and make evident the value and outputs of the service,

You will coach and support the Community Matrons in their responsibilities in supporting our neighbourhood nursing teams, in developing their capacity for self-management, coaching and leadership.

This is an exciting time to join our community nursing services., We have recently been awarded 'Outstanding' by CQC and we are converting our District Nurse teams to a brand new model of nursing, introducing the new Associate Nurse role and mobile working. This is a brand new post for a fixed term of one year (in the first instance) in which you can work with us to shape and develop our fantastic Community Matron team.
