Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their Prevention team.

Responsibilities of this role:
- Undertake strengths-based assessments, reviews, and support planning with adults and their carers.
- Promote well-being, independence, and safety, enabling individuals to live in the community and avoid unnecessary hospital admissions.
- Apply Care Act (2014) principles and other relevant legislation (Mental Capacity Act, Mental Health Act).
- Work collaboratively with individuals, families, and community networks to reduce isolation and build resilience.
- Use a strengths/asset-based approach in care planning and team culture.
- Liaise with health professionals, voluntary sector, and other agencies to deliver integrated support.
- Undertake safeguarding enquiries and respond to concerns about adults at risk.
- Maintain accurate case records, reports, and documentation in line with statutory requirements.
- Participate in community engagement initiatives to prevent marginalisation and promote inclusion.
- Contribute to service development and innovation, embracing change and co-production with service users.
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 3 years post qualifying social work experience
