Care Home Liaison Officer

We are seeking a dedicated Care Home Liaison Officer to join our team based in Wakefield. This role is crucial in supporting the Hospital Social Work Team to facilitate safe and timely hospital discharges to care home settings across the Wakefield district and beyond. The position involves providing essential data, coordination, and administrative support within a community hospital setting.
Day-to-day of the role:
- Provide data support to the Integrated Discharge Team and other health and social care colleagues.
- Assist Hospital Social Work Team Managers with business intelligence to support safe and timely hospital discharges to various care settings.
- Liaise with care facilities across the Wakefield district to ensure effective, timely, and safe transfer of care from the hospital.
- Support the Hospital Social Work Team with complex care placements and facilitate safe transfer of care to other settings.
- Utilise IT skills, especially in Excel, to present real-time performance data.
- Ensure adherence to data protection, confidentiality, and information security principles.
Required Skills & Qualifications:
- Strong IT skills, particularly in Excel, for data presentation.
- Knowledge of data protection, confidentiality, and information security principles.
- Experience in a similar role, dealing with sensitive and confidential enquiries efficiently.
- Proven ability to work in a fast-paced environment and meet deadlines.
- Experience in multi-agency working and handling sensitive information relating to adult social care.
- Ability to input, interrogate, and validate data from various sources to produce documents and reports.
