Role Overview:
We are seeking an enthusiastic and experienced Social Worker to join the Early Intervention and Prevention team in Stockton-on-Tees. In this role, you will work proactively to identify and address emerging social care needs, providing support and intervention to individuals and families to prevent crises and improve outcomes.

Key Responsibilities:

Undertaking assessments of individuals and families referred to the Early Intervention and Prevention service, identifying strengths, risks, and support needs.
Developing and implementing tailored support plans in partnership with service users, families, and relevant agencies, focusing on early intervention and prevention strategies.
Providing practical and emotional support to individuals and families, signposting to appropriate services and resources to address identified needs.
Collaborating with partner agencies, including health professionals, schools, housing providers, and voluntary organizations, to coordinate holistic support packages for service users.
Delivering targeted interventions and group work sessions to address specific issues such as parenting skills, mental health awareness, and substance misuse prevention.
Monitoring and reviewing the effectiveness of support plans, adjusting interventions as necessary to ensure positive outcomes for service users.Requirements:

Qualified Social Worker registered with Social Work England.
Experience working within a social care setting, preferably in early intervention and prevention services.
Ability to work effectively with individuals and families from diverse backgrounds, demonstrating empathy, sensitivity, and cultural competence.
Knowledge of relevant legislation, policies, and procedures relating to social care, safeguarding, and early intervention.
Commitment to promoting independence, resilience, and well-being through early intervention and prevention approaches.Benefits:
