Job Description

We are seeking a Certified Medical Assistant to become a part of our Clinical team. You will assist patients to optimize their medications under the direction of a clinical pharmacist and the patient's primary care physician.


  1. Assembly of full medication list for each consented patient, to include prescription drugs, OTC, Vitamins, herbals and any other item being taken/used for therapeutic purposes
  2. Establishment of medication utilization, prescribed rate and dispensed rate
  3. Preparation of full prescriber list for each patient
  4. Notification of all prescribers of forthcoming review of the patient's medication
  5. Preparation of full pharmacy list for each patient and linkage between each pharmacy and each medication dispensed
  6. Recording of patient's primary and secondary pharmacy insurance with clear delineation of any narrow network or other cost barriers for the patient
  7. Assessment of potential coupon, voucher or other discount available to the patient for medications on the prescribed list
  8. Preparation of completed chart prep not less than 48 hours prior to the patient appointment
  9. Patient reminder call 24-48 hours prior to appointment
  10. Documentation of intervention and care plan in the practice EMR and any other software systems applicable
  11. Cancellation of all outstanding prescriptions in discontinued pharmacies
  12. Synchronization of medications on instruction from the physician and preparation of dispensing record for approval and transmission
  13. Preparation of patient synchronization instructions
  14. Maintenance of appointment schedule for follow-up appointments
  15. Verification of successful passage of synchronized prescriptions through dispensing pharmacy
  16. Monitoring for and notification of emergent or new prescriptions
  17. Maintenance of therapeutic goal records
  18. Management of patient relationship in collaboration with clinical pharmacist and physician, including frequent telephone interaction with patient
  19. Such other associated tasks that may arise from time to time at the instruction of the clinical pharmacist


  • Certified Medical Assistant or Pharmacy Tech Certification
  • Previous experience in primary care, pharmacy or other related fields
  • Strong analytical and critical thinking skills
  • Excellent written and verbal communication skills
  • Strong attention to detail
  • Strong organizational skills
