Buurztzorg Model Community Nurse

We require a Nurse with prescribing (minimum V150) to support Bermondsey Neighbourhood Nursing team. The post holder will have experience in assessing patients and prescribing from the Nurse formulary.

We are seeking dynamic Nurses with community experience to join us here at Guys and St Thomas'. We have led the way in testing new community models of nursing and are the first trust in the UK toapply the Dutch Buurtzorg approach to community nursing.

We are now ableto work in this new model and are rolling out across community,this is a fantastic opportunity for you to help shape community nursing for the future and to be part ofworking inthis brand new way of working in community nursing in the UK.

We are looking for Nurses who work well in a team, can think outside the box, loves going the extra mile, thrive on a challenge and are open to Buurtzorg's approach in the UK Community Nursing field.

This is a fantastic opportunity particularly for current District Nurses wanting to work in a different way where they get more time with their patients in a more holistic manner.

The benefit to the patient with this new approach is that the team in place identifies solutions quicker for the patient, improves independence and streamlines care more effectively. This personalized attention and team approach allows individuals to stay in their homes and communities for as long as possible, avoiding hospital admission.

The benefit to the staff member is the professional freedom and responsibility that comes with supervising and supporting each other in self-managed teams. Coaches not managers support the team and the Nurses will decide how the care will be provided to their patients. This encourages flexibility and improves staff satisfaction.

Please Note : You will move over into the role at your current NHS spine point on Agenda for Change - this means, if you are a band 5, you will move at your band 5 spine point. You wil have opportunity to develop into a band 6 within the model.
