Position Overview
We are currently looking for a qualified and motivated Social Worker to join our agency at Sirona Medical Social Work department. You will be expected to deliver a high quality social work service to older people, and their families/carers, who are deemed vulnerable or needing support. They will need to work with the citizen, carers and families to promote independence and provide support where necessary, ensuring that the citizen, families and carers are at the centre of decision making about their lives and the outcomes that they want to achieve.
Qualifications

BA Honors or Master's Degree in Social Work
Registered with Social Care Wales.
Eligibility to work in the UK
Full UK driver's license and access to vehicle (if applicable)
Completed Assessed and Supported Year in Employment (ASYE)
Minimum of 3 years post qualified experience 
Team work & interpersonal skills
Strong interpersonal and communication skills, with the ability to work in a fast-paced healthcare environment.
Empathetic, patient-centered approach with a dedication to providing high-quality care.
Ability to work collaboratively within a multidisciplinary team
The assignment is for 37HPW, Monday - Friday. There is an expectation that candidate lives locally or with in a commutable distance as the position is located in Conwy County.
