Family Practice physician employment in Illinois : Immediate need for a Family or Internal Medicine Physician to join a well established Primary Care group. Lets Discuss Loan repayment Multiple opportunities / multiple locales 40K sign on bonus J1 visa sponsorship H1B visa sponsorship Position Functions: Provide the full scope of services which fall under providers field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines contained within the quality management, risk management, infection control, customer service and safety programs Duties and Responsibilities: Obtains health history and perform physical examinations. Diagnoses, treats, counsels, and coordinate care for each patient. Reviews and update problem list and longitudinal plan of care at every visit. Meets documentation standards and completes all progress notes within 24 hours following the patient visit. Fulfills on-call responsibilities in accordance with the schedule developed by our group and responds to calls within 20 minutes. Prescribe medication in accordance with Illinois statute and professional practice guidelines. Performs all duties in conformity with the quality standards and clinical guideline established by our group Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security. Manages EHR In-Basket, transfers In-Basket responsibilities to colleagues when appropriate, and responds to messages (e.g. lab results, refills, patient calls, patient messages, co-sign orders, co-sign charts, and open charts) within 72 hours. Recognizes patients families as an integral part of patients care teams. Ensures that patients and families needs are met and clinic policy is followed through. Encourage patients and families to voice questions and/or concerns. Works with patients and their families as appropriate to encourage care self-management. Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment, and reinforcement of providers follow-up instructions. Utilizes printed health education materials as appropriate and document all instructions given; while giving consideration to an individual varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care. Monitors and ensure the cost-effective utilization of clinical resources. Works with care team and case management to provide for coordinated delivery of case management services. Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately. Meets intensity of service and/or severity of illness admission criteria. Provides triage services for same-day appointment patients, identify those patients who can wait to receive care and those patients who require immediate medical attention through consultation with patients and/or their families. Schedule services for the patient within a time frame meeting the patients medical needs. Coordinates with health center staff at all levels to ensure patient needs are met. Exhibits a positive attitude during daily activities and fosters a positive work environment for the entire team. Participates in health center huddles and meets scheduled health center times and productivity standards. Assures patients are seen in a timely and clinically appropriate manner, and involves health center employees including medical assistants and front desk staff in this process. Assures care teams properly carry out effective treatment, education and performs clinical tasks in accordance with acceptable policy and procedure. Assures all clinical activities are accomplished according to acceptable guidelines for reimbursement. Participates in the orientation and training of staff as needed. Ensures care team meets internal certification requirements for patient education and the administration of medication. Participates in proficiency testing as required by our group to ensure that proper standards of care are being maintained. May function as a preceptor in teaching programs including medical residents, medical students, advance practice nurse students as well as other allied health professions. Follows our protocols for research projects. Adheres to Federal Tort Claims Act regulations (e.g. care for our patients only (unless dictated by hospital bylaws, emergencies, or our approved events health fairs or others - and adheres to scope of practice determined by our credentialing / privileging process). Other duties as assigned. Primary Care IM/FM comp range Family Medicine- 145-160K depending on experience and a $40,000 sign-on bonus. A productivity bonus benchmark up to $5,000 year and a productivity bonus $3, 000 per 150 encounters and a Quality Bonus up to $6000. Internal Medicine- 155-170K depending on experience with a $40,000 sign-on bonus. A productivity bonus benchmark up to $4,600 year and a productivity bonus $4, 000 per 150 encounters and a Quality Bonus up to $6,000.
