Tripod Partners are looking for a Continuing Healthcare Nurse Assessor (RNLD / RMN / RGN) to join a CHC team based in Kettering. The role involves facilitating multi-disciplinary assessments and patient reviews, managing CHC cases, and ensuring adherence to the National Framework.

Key Responsibilities:

Conduct CHC assessments, reviews, and eligibility investigations

Manage a caseload of CHC patients and support discharge pathways

*

Collaborate with MDTs, care providers, and families to ensure safe, appropriate care planning

*

Act as Duty Clinician when required

*

Ensure timely documentation, compliance with data protection, and accurate reporting

*

Participate in training, service improvement, and quality assurance activities

*

May involve elements of dispute resolution, transitional care, and safeguarding work

Rate: £35 per hour
Contract: 2 Months Initially
IR35 Status: Inside IR35

Requirements:

*

Registered Nurse with valid NMC PIN (RNLD / RMN / RGN)

*

CHC experience or relevant adult/LD mental health assessment background

*

Excellent knowledge of CHC framework, safeguarding, and capacity/consent principles

*

Strong organisational and communication skills

*

Car driver with access to own transport

Working Pattern:

*

Monday to Friday

*

On-site role with county-wide travel
