Job PurposeA qualified social worker is required for the hospital discharge to assess team South. The post is based in Wythenshawe hospital, south Manchester and involves supporting patients in both Wythenshawe hospital and Trafford General with arranging safe and timely discharges back to their own home or 24-hour care setting.
