Leaders in Care is proud to be recruiting on behalf of a respected, nurse-led care provider delivering high-quality, person-centred services to children and adults in their own homes across South East London.

This is an excellent opportunity for an experienced and organised Care Coordinator to join a growing service, working closely with the Business Development and Mobilisation teams to support the setup and delivery of new care packages.

The Role As a Care Coordinator, you will play a key role in coordinating new and existing packages, supporting the safe mobilisation of services and ensuring smooth day-to-day operations.

You will work closely with the Business Development team during the onboarding of new packages, as well as the Mobilisation team to ensure services are set up efficiently, safely and in line with governance requirements.

Key Responsibilities
Coordinate new care packages from referral through to mobilisation and ongoing delivery
Work closely with Business Development and Mobilisation teams to support new package setup
Manage rotas and ensure safe deployment of care teams
Complete care plans, risk assessments and service documentation in line with CQC requirements
Liaise with families, clinicians, carers and internal teams
Ensure accurate record keeping and compliance across all packages
Support smooth transitions and onboarding of new service users
About You To be successful as a Care Coordinator, you will demonstrate:

Previous experience in a Care Coordinator role within community complex care or domiciliary care
Strong organisational, communication and time-management skills
Experience supporting mobilisation or onboarding of care packages (desirable)
Good understanding of CQC and regulatory requirements
A proactive, solution-focused and adaptable approach
