We are seeking an experienced Senior Social Worker to join the Wolfson Neuro-Rehabilitation Unit at Queen Mary's Hospital. This is a key role for a professional with a strong background in adult social care, hospital discharge planning, and neuro-disability.
The Wandsworth Hospital Team works closely with the Wolfson Unit to provide a high-quality, person-centred discharge service that promotes independence and well-being for patients and their carers. You will support individuals with complex physical and cognitive needs and play a central role in longer-term care planning.
What We're Looking For:

Qualified Social Worker registered with Social Work England.
Extensive post-qualification experience in adult social care, ideally within hospital or neuro-rehabilitation settings.
Proven ability to manage complex cases and lead discharge planning.
Experience supervising staff and contributing to service improvement.
Strong knowledge of relevant legislation and statutory responsibilities.
Excellent communication, decision-making, and organisational skills.
Collaborative approach with confidence in working across health and social care disciplines.Key Responsibilities:

Provide supervision and support to Social Workers in the team.
Lead complex discharge planning and support patient flow within the Wolfson Unit.
Undertake assessments, manage risk, and make evidence-based decisions.
Act as safeguarding enquiry officer and manage new referrals.
Attend and contribute to multidisciplinary meetings.
