About the Role
As a Social Worker in the Assessment Team, you'll be the first point of contact for adults seeking support. Your responsibilities will include:

Conducting Care Act assessments to identify care and support needs

Developing person-centred care plans that promote independence

Collaborating with health professionals, housing services, and community organisations

Utilising a strengths-based approach to empower residents

You'll work within one of our three locality teams:

East: Tottenham

Central: Wood Green, Harringay, West Green

West: Hornsey, Alexandra Park, Crouch End, Highgate, Muswell Hill

About You
We're looking for Social Workers who:

Have a solid understanding of the Care Act 2014 and related legislation

Are skilled in conducting assessments and developing care plans

Can work collaboratively with a range of stakeholders

Are committed to promoting independence and wellbeing
