Salary: £21892 - £24157/annum

Our client will be running a virtual assessment centre for patients with post covid symptoms.

Reporting to the lead nurse you will handle administration for the team as well as undertaking virtual assessments of patients using questionnaires and guidelines

Duties will include:

Undertake virtual assessment including a full range of observations, recording findings and escalating concerns as required.
Undertake virtual baseline assessment of patients clinical condition via agreed screening questionnaire
To arrange and input multi-Disciplinary meetings to the clinical system, ensuring relevant clinicians available.
Ensure all relevant patient information is available and accurate
Communicate Multi-Disciplinary meeting outcomes.
Assist with the follow-up on all actions / outcomes at each stage of the patient's pathway.
Onboard patients onto the 'Your Covid Recovery' App.
Assist in the monitoring of patents virtually through the App dashboards - including review of data entered by the patients and providing advice to the patient or escalation as required on a weekly basis.
Follow agreed clinical protocols referring to senior colleagues or GP as appropriate.
Provide interventions and treatment to patients in line with service policy and using established clinical guidelines.
Educates and advises patients regarding assessment and management and when they should contact a doctor in line with guidance.
Ensure infection, prevention and control guidance is applied in practice.
Ensure equipment is available for patients who require virtual monitoring in their homes.
Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.
Record clear and contemporaneous IT based consultation notesIt is essential that you are an experienced administrator from a healthcare background. This could either be by having an NVQ level 2 + in Health & Social Care or previous experience in a healthcare or clinical environment.
