Experienced Occupational Therapist - ReablementLocation: South West EnglandContract: Full TimeSector: Adult Social CareAn experienced Occupational Therapist is required to join a busy Reablement Team within a local authority Adult Social Care service. The team focuses on supporting individuals to regain independence and reduce the need for long-term care.Key Responsibilities

• Manage a complex caseload of adult clients
• Complete occupational therapy assessments and develop reablement plans
• Carry out moving & handling and risk assessments
• Recommend equipment, adaptations and Disabled Facilities Grants
• Work collaboratively with health professionals, families and partner agencies
• Provide professional support and guidance to colleagues
• Maintain accurate records and reports

Requirements

• Degree or Diploma in Occupational Therapy
• HCPC registration
• Minimum 2 years post-qualification experience
• Experience working with adult clients and complex needs
• Knowledge of health, housing and social care legislation
• Experience with adaptations, reablement and moving & handling

Additional Information

• Enhanced DBS required
• Travel required for home visits
