About the Role:

As a Social Worker in the Children in Care/Leaving Care Team, you will play a pivotal role in supporting children in long-term care, whether through Care Orders or Section 20 arrangements. Your responsibilities will include progressing long-term care planning, facilitating pathway planning, and supporting young people during their transition to adulthood. Working closely with families, foster carers, residential settings, and semi-independent settings, you will provide crucial assistance to meet the diverse needs of the children.

Key Responsibilities:

  • Utilize your knowledge and experience of statutory frameworks, policy and procedure, and best practice to ensure the highest quality of care and support.
  • Demonstrate proficient IT skills to efficiently manage caseloads and data.
  • Embrace a high level of determination, organizational skills, personal drive, energy, and ambition to achieve challenging targets and improve outcomes.
  • Employ highly refined interpersonal skills to establish and maintain productive relationships with colleagues, partners, agencies, and, most importantly, young people and their families.
  • Conduct assessments to identify the needs of children and devise appropriate intervention plans.
  • Collaborate effectively with multi-agency networks to ensure holistic care and support for the children in your care.
  • Handle court work under the Children Act 1989 & 2004 with professionalism and diligence.
  • Strive towards achieving permanence for children, securing stable and nurturing environments for their development.
  • Actively contribute to the ongoing development and improvement of our Children in Care Team.

Person Specification:

Qualifications:

  • Qualified Social Worker with a recognized professional qualification (e.g., BA Social Work, MA Social Work, DipSW).
  • Current registration with Social Work England.
