About the RoleWe're recruiting an experienced and passionate Team Leader for Targeted Support to join Hounslow Council. In this pivotal role, you will lead a team responsible for assessing and supporting vulnerable residents with complex needs-helping them avoid crisis, maintain independence, and reduce reliance on statutory services.

Key ResponsibilitiesAs Team Leader, you will:
- Manage an effective Floating Support Link Team, ensuring accurate assessments and appropriate referrals to Floating Support, Community Solutions, VCS partners, and other services.
- Work closely with the commissioned provider to understand service capacity, manage caseloads, and maximise the benefit of targeted support.
- Oversee processes for clients and referral partners, championing a high-quality service and gathering feedback for continuous improvement.
- Represent the service at senior, strategic, and performance meetings.
- Act as the key point of escalation for complex or safeguarding-related cases.
- Use systems such as LAS and Granicus to manage cases and track outcomes.
- Analyse data and insight to identify trends, monitor performance, and proactively address service issues.
- Work collaboratively across the Council-including Joint Commissioning, Housing, and the Community Partnership Unit-to help shape support pathways and future commissioning.
- Pilot innovative, resident-focused approaches with internal teams and the Floating Support provider.
- Build strong working relationships with the voluntary and community sector, identifying gaps in provision and supporting capacity building.
- Promote the service to key referral partners such as GPs, social care teams, hospital discharge teams, mental health and substance misuse services, and probation.

Skills & ExperienceRequired:
- Demonstrable experience supporting and assessing vulnerable people with complex needs.
- Strong decision-making and leadership skills.
- Experience working with multi-agency partners or complex caseloads.
- Ability to work strategically and operationally within a fast-paced environment.
- Strong communication skills - this is a customer-facing role, requiring a level of spoken English that meets the statutory Code of Practice under the Immigration Act 2016.
- Enhanced DBS check (required).

- Ensure residents experience a smooth transition into, through, and out of the service - helping them sustain independence.
- Work proactively with Council teams and partners to forecast demand and support effective service planning.
- Contribute to wider Council objectives by working collaboratively and strategically across departments.
