Salary: £24 - £28 per hour DOE

Are you an experienced and motivated Clinical Pharmacist looking to make a real difference in primary care? We're looking for a proactive and patient-focused pharmacist to join a supportive and forward-thinking GP practice in Bradley Stoke.

Key Responsibilities:

* Conduct medication reviews and optimise prescribing in line with best practice and local guidelines

* Support chronic disease management (e.g. hypertension, diabetes, asthma)

* Handle medication queries from patients and clinical staff

* Assist with repeat prescription authorisations and medication audits

* Collaborate with GPs, nurses, and wider primary care team to deliver integrated patient care

* Provide expert advice on medicines management and safety

Requirements:

* GPhC registered Pharmacist

* Experience in a primary care or GP surgery setting (desirable)

* Independent Prescriber qualification - (desirable)

* Strong communication, clinical, and IT skills
