About the Role:

We are seeking a dedicated Social Worker to join our dynamic multi-disciplinary team, providing crucial support to individuals recovering from neurological conditions. In this role, you will play a vital part in:

Conducting comprehensive social work assessments for individuals with neurological conditions, focusing on their social, emotional, and practical needs.
Developing and implementing personalised care plans in collaboration with service users, families, and other rehabilitation professionals.
Providing emotional support, guidance, and advocacy to service users and their families navigating the complexities of neurological rehabilitation.
Connecting individuals with appropriate community resources, support networks, and long-term care solutions.
Working collaboratively within a multi-disciplinary team (including physiotherapists, occupational therapists, speech and language therapists, and nurses) to ensure holistic and integrated care.
Safeguarding vulnerable adults and ensuring compliance with all relevant legislation and best practice guidelines.

About You:

You will be a Qualified Social Worker with:

Experience working with adults, ideally within a rehabilitation, disability, or health setting.
A strong understanding of the impact of neurological conditions on individuals and families.
Excellent assessment, communication, and organisational skills.
A commitment to promoting independence and enhancing the quality of life for service users.
Current Social Work England registration.Rate: £32.00 Umbrella
