Job Title: Primary Care Network Care Coordinator 

Focus: Community engagement and project managers - elderly care 

Contract Type: 6 months fixed term

Hours: 37.5 per week with opportunity for paid over-time

This role is working for a 'Primary Care Network' (PCN) called Neohealth. A PCN is a group of GP clinics who work together delivering various areas of primary care. Neohealth play an important role in their community, supporting people from all kinds of different backgrounds and are like one big family, which you will join as a Care Coordinator. 

As a Care Coordinator you will: 

  • Engage with the community and contact patients to book appointments
  • Communicate with other GP practices within the PCN to coordinate appointment availability and care pathways 
  • Project manage and coordinate the booking of appointments across a range of areas of primary care including: complex health, learning disabilities and elderly care 
  • Planning and coordinating appointment slots with availability of GPs
  • Follow up with patients 2 days before their appointment to remind them to attend
  • Using 'emis Systm one' to access and update patient records (training on this is available).
  • Working and liaising with a multidisciplinary team

Mode of work:

  1. This is initially a 6-month fixed term contract, working from the Notting Hill Medical Centre
  2. After the fixed term contract there is the opportunity to join on a permanent basis and continue to develop your career with the NHS 
  3. 37.5 hours a week with the option to take on paid over-time 
  4. Option to work 4 or 5 days per week
  5. Laptop and phone provided 

What we need from you:

  • Excellent communication and telephone skills
  • Be comfortable in talking to people from all walks of life and backgrounds 
  • Good organisational and time management skills
  • Experience in a similar healthcare / administration position is desirable
  • Ideally be proficient in using emis System One
  • Ideally have experience in elderly care 
  • Be a self-starter and take incredibly busy days in your stride!
  • Flexibility with working hours
