About the Role:
Liquid Personnel is recruiting for an Adult Social Worker to join the fast-paced Hospital Discharge Team in Darlington. This is a fantastic opportunity to make a real difference by supporting timely, person-centred discharges and empowering individuals to regain independence in their communities.
What will your responsibilities be?
In this role, you will support hospital discharges by reviewing individuals placed in temporary residential care following discharge, using a Home First approach that prioritizes recovery at home. You will apply a strengths-based model and person-centred planning in line with the Care Act 2014 to assess needs, promote independence, and coordinate timely, safe transitions from hospital to community settings.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
