Good Afternoon,

I am currently representing Islington Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience

We are looking for a Occupational Therapist this role will be: N1 1XR

The right candidate will:

* To carry a caseload of varied range of complexity.

* To take on care coordination responsibility for their caseload.

* To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005

* To ensure that the Care Act 2014 is implemented in all aspects of their role.

* To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.

* To obtain informed consent for all care coordinated cases.

* To ensure that their Senior Practitioner is kept informed of any significant events relating to the service.

* To provide advice and consultancy to colleagues within the immediate team as well and to a wider range of stakeholders, which may include the NHS, the Independent sector and members of the public as appropriate.

* To keep records of work undertaken and entering data on the database in accordance with Council Policy.

We require the following:

* Diploma in Occupational Therapy or other equivalent qualifications

* Experience of assessing the needs of adults and a knowledge of the services available to meet their needs

* A working knowledge of the key principles of the Care Act 2014 and other relevant Health and Social Services legislation relating to the provision of services for all adult client groups.
