Hunter Mental Health is supporting an amazing older adult community mental health service within a fantastic NHS Trust which boasts providing the highest quality integrated mental health, community and learning difficulties health services.

We are looking for an experienced RMN or CPN with prior experience working with adults or older adults to join this service on a 37.5-hour locum contract which is an initial 3-month contract with a strong intent to extend further. The pay range we are offering is £27 - £30ph based on experience. Some flexibility is possible for 4 long days, Some WFH will be possible once settled in.

JOB PURPOSE

To promote the principles of CPA with the Getting More Help Service.
To embed and review the development of transitional, developing and existing care pathways.
To develop care pathways, and sign- post young people to appropriate mental health services.
To provide assessment and intervention for children and young people experiencing serious mental illness.
To participate in the team Duty Rota.
To provide consultation, advice, training and support to partnership agencies and members of the multidisciplinary team
DUTIES AND RESPONSIBILITIES

Main Performance Areas.

To develop and provide a high standard of assessment and treatment for patients referred to the service.     
To take responsibility for an agreed clinical caseload, involving care co-ordination as appropriate to ensure continuity.
To collaboratively assess and provide evidence-based treatment programmes and discharge plans utilizing CPA for patients.
To plan and provide appropriate therapy services in consultation with users, Carers and clinical teams for patients referred, within defined resources and using evidence-based practice. 
To develop and maintain relevant and evidence-based therapeutic activities as appropriate to the patient's needs.
To regularly review and evaluate treatment programmes with patients, carers' and members of the clinical teams.
To participate in clinical meetings, case conferences and CPA meetings.
To enable the transition of patients through stages in their care involving care planning and therapy programmes.
To compile written reports as required and to generate and maintain patient documentation.
To monitor and evaluate the service provision.
To take part in the Clinical Duty Rota. To complete urgent and emergency assessments. Within the Clinic and community settings.
The service is looking for someone to work within their working age adult adult team working with patients aged 18 - 65, operating in a 9-5, Monday to Friday service. Within the service there is a support team of CPN's, OT's and Psychiatrists, along with the supervision of the Team Lead.
