Randstad is currently recruiting for a Qualified Social Worker to join Liverpool City Council's Preparing for Adulthood Team. This is a full-time position offering an excellent hourly rate of up to £43 per hour DOE.

Responsibilities:

• Support young people (aged 14-25) with special educational needs, disabilities, or care backgrounds as they transition from children's to adult services.
• Develop and review person-centred transition plans.
• Carry out Care Act assessments and identify eligible needs.
• Create support plans that promote independence and positive outcomes.
• Work closely with education, health, housing, and employment services.
• Attend EHCP reviews and multi-agency transition meetings.
• Identify and respond to safeguarding concerns under relevant legislation.
• Promote choice, control, and involvement in all planning and decision-making.
• Build positive relationships with young people and their families or carers.
• Monitor and review support plans, adjusting as necessary.
• Maintain accurate and timely case records in line with policy and statutory requirements.

Requirements:

• Registration with Social Work England
• A recognised Social Work degree or equivalent qualification
• Enhanced DBS check
