Our client is looking for someone to work in their Wellbeing Support Service. You’ll be working closely with the Adult Social care team to support vulnerable customers to gain local community support to enable independent living in a place they call home. The role will include ad hoc or regular visits to people identified in the community as needing further support and complete tasks as requested. Examples of tasks might be checking homes are warm and safe, supporting customers to open and deal with correspondence, supporting customers to appointments, prompting medication and ensuring food is available. You will be supporting a range of clients including those with disabilities, physical health problems, dementia, substance misuse, mental health and neurodiversity.
Job requirements
- Providing an emergency food parcel from their own stock or a food bank whilst arranging long term support.
- Completing a partner referral form for a financial assessment.
- Carry out a holistic assessment with customers using an outcome star to assess eight different areas of their lives and complete an action plan to ensure they are happy, safe and well in their home.
- Connecting customers with community lounges to support their needs.
- Completing detailed accounts from the visits and recording any interventions, signposting and contact records.
- Reporting back into adult social care daily with notes from the visits.
- Raising a safeguarding where applicable.

- Excellent communication skills, showing compassion and empathy.
- Understanding and respecting individual needs while providing personalised support in a positive and supportive manner
- Support worker experience is desirable but full training will be given.
- An aptitude for working with vulnerable and/or older people
- Ability to work under pressure and manage your daily tasks
- Excellent ICT skills
- Excellent administrative skills
- Excellent knowledge of local services including national and local charities
- Be educated to GCSE level.
- Be confident and resilient and able to problem solve and think quick on your feet, in this role you’ll face challenges, so you’ll need to be able to adapt.
- Be able to motivate yourself and work independently and plan your time effectively.
- Hold a full UK driving license with use of your own vehicle including business insurance.
- Carry out a DBS check.
