Our Client, a Greater Manchester council, is seeking an Adult's Social Worker to join their Integrated Care Team.

Responsibilities of this role:
- Undertake strengths-based assessments under the Care Act 2014
- Develop and review person-centred support and care plans
- Work collaboratively within multidisciplinary integrated care teams
- Coordinate care with health professionals, voluntary sector, and community services
- Manage a varied caseload, including complex and high-risk cases
- Safeguard adults at risk in line with statutory duties and local procedures
- Support hospital discharge planning and prevent unnecessary admissions
- Promote independence, choice, and wellbeing for service users
- Complete mental capacity assessments and best interest decisions
- Maintain accurate, timely case records and statutory documentation
Requirements of this role
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 3 year's experience working as a Qualified social worker
