Conwy County Borough Council is seeking an experienced and motivated Social Worker to join our Children, Families & Safeguarding Service, working alongside our Mental Wellness Team. This is an excellent opportunity for a skilled practitioner looking to contribute to high-quality, person-centred social work practice within a supportive and collaborative environment.
About the RoleYou will support the delivery of social work services to individuals who are vulnerable or experiencing prolonged social stressors that impact their day-to-day functioning. Your role will involve undertaking assessments, planning interventions, and supporting individuals to improve resilience, independence, and emotional wellbeing.
A key part of the post will be working closely with third-sector organisations and partner agencies to ensure that responses are proportionate, strengths-based, and tailored to each person’s needs.
You will work directly with individuals, carers, and families, ensuring their voices, goals, and outcomes remain central throughout all stages of assessment and intervention.
Key Responsibilities:- Deliver high-quality social work support to vulnerable adults and families.
- Undertake assessments and develop person-centred care and support plans.
- Apply strong safeguarding knowledge and decision-making in complex cases.
- Build effective partnerships with third-sector and statutory organisations.
- Promote resilience, independence, and wellbeing for individuals and families.
- Maintain clear, accurate, and compliant case records.
- Ensure individuals, families, and carers remain central to all decisions and planning.
- Work on-site at Coed Pella a minimum of several days each week to support team activity.
Skills & Qualifications Required:- Qualified Social Worker with a minimum of 3 years post-qualified experience.
- Registered with Social Care Wales.
- Experience working with vulnerable adults (18+), mental health, or safeguarding.
- Working knowledge of the Mental Health Act 1983.
- Strong interpersonal and team-working skills.
- Confident with technology and social care case management systems.
- Able to commute to Conwy County and work from the office as required.
