Practice Nurse

Location: WF13 1HN

Pay: £25–£29 per hour

Schedule: 3 days per week – Monday, Wednesday & Friday, 10:00–16:30

System: SystmOne

Service: New trial service (potential permanent role)

We are seeking a dedicated Practice Nurse with strong wound care experience to support this innovative wound care pathway. You will deliver high-quality clinical care, support the new service model, and ensure evidence-based wound management is delivered consistently.

The Health Centre has been selected as the Test and Evaluation Site (TES) for the AHSN Network’s Transforming Wound Care Programme—the first site in Yorkshire and the Humber to join this innovative initiative.

You will be part of a service focused on:

• Faster healing of wounds
• Improved quality of life for patients
• Lower recurrence of chronic wounds
• More efficient use of health and care resources

Key Responsibilities

• Provide wound care for patients with lower limb wounds
• Assess, plan, and implement wound management in line with national guidelines
• Work collaboratively with multidisciplinary teams
• Ensure accurate documentation using SystmOne
• Support service development during the test & evaluation phase
• Promote patient education and self-management

Requirements

• Experienced Practice Nurse
• Strong wound care experience
• Level 1 or Level 2 Wound Care training
• Familiarity with SystmOne (desirable)
• Passion for reducing health inequalities and improving community care
