About the Role:We are seeking a dedicated and organised Care Coordinator to join our domiciliary care team. You will play a vital role in ensuring the smooth and effective delivery of high-quality home care services. The ideal candidate will have a passion for supporting vulnerable adults and the ability to manage care schedules, communicate effectively with carers and service users, and uphold compliance with CQC standards.
Key Responsibilities:- Coordinate daily care visits for clients, ensuring rota coverage and continuity of care.
- Liaise with care workers, service users, families, and external agencies to maintain high standards of service.
- Ensure care plans and risk assessments are accurate, up-to-date, and regularly reviewed.
- Respond promptly to any issues or changes in care needs, including emergency cover arrangements.
- Support recruitment, onboarding, and training of new care staff as required.
- Participate in the on-call rota, providing out-of-hours support as needed.
- Ensure compliance with CQC regulations and company policies.
- Maintain accurate records using care management systems.
- Conduct spot checks, supervisions, and quality assurance visits in the community.
Requirements:Essential:
- Previous experience in a care coordination or domiciliary care role.
- Excellent communication and organisational skills.
- Strong understanding of scheduling and rostering systems.
- Ability to work under pressure and meet deadlines.
- Full UK driving licence and access to a vehicle.
- Good working knowledge of CQC regulations and safeguarding procedures.
Desirable:
- NVQ Level 3 in Health & Social Care (preferred).
- Experience with electronic care management software
- Local area knowledge is an advantage.
