Why This Role is Important to Us

Commonwealth Care Alliance s clinical pharmacist will provide pharmacy
support for high risk CCA members within a designated clinical team. The
pharmacist will improve member s care via outreach, a pharmacy-based
assessment, and education within the member s home, CCA primary care
practice, or telephonically. As an integrated team member, the pharmacist will
have the primary goal of reducing admits/1000, readmission rates, and
polypharmacy. The pharmacist will improve the medication management of members
by addressing gaps in care, quality improvement opportunities, and patient
safety measures as defined by leadership, CMS, or MA Health.

The pharmacist will ensure optimization of medication related healthcare
outcomes and patient safety while simultaneously recommending cost-effective
medication strategies.

What_You'll_Be_Doing
* Work collaboratively within an interdisciplinary team to provide
transitions of care support to members by conducting medication reviews,
assessments, education, and medication management support to prevent
readmissions within the ambulatory (e.g. HF/COPD) populations.
* Provide pharmaceutical care to targeted complex, high risk members within
home, CCA primary care practices, or telephonic based on need and
identify drug optimization issues, polypharmacy, drug related problems
(DRP), gaps in therapy to prevent medication errors, potentially
avoidable admissions and reduce ER utilization
* Demonstrate improvement in key performance indicators (KPI), process, and
outcome metrics as defined by leadership
* Partner with prescribers and interdisciplinary providers to resolve any
medication related problems, polypharmacy, or other related concerns.
* Review pharmacy utilization reports to determine appropriate plan for
pharmacy management, as applicable.
* Operate in an ambiguous environment without continuous supervision, while
maintaining productivity and self-accountability
* Demonstrate collaborative team behavior, effective communication, and
coaching for support staff by advising of assessment, change
recommendations, and identified concerns.
* Support pharmacy initiatives to improve quality outcomes while lowering
medication costs to organization (members have no cost share)
* Improve member and provider experience
* Participate in the development and presentation of educational lectures
for clinical team and CCA staff as required
* Serve as a resource for CCA clinicians to field medication therapy
questions
* Participate in root cause analysis reviews and interdisciplinary team
meetings
* Understand and present reports on drug utilization and cost to providers
and management as necessary
* Document clinical notes in a clear, effective manner
Must be willing and able to travel to member s homes, CCA offices, or withi
designated geography
Must have a valid driver s license with no restrictions; ability to be acti
and mobile

What_We're_Looking_For

MA Board of Pharmacy Licensure required
Degree in Doctor of Pharmacy (Pharm D.) required
Board certified in BCACP, BCPS, or BCGP preferred
ASHP accredited residency preferred

2+ years clinical pharmacy experience or residency required

Managed_Care_Experience_Preferred

Ability to prioritize workload and manage multiple cases simultaneously.
Ability to problem solve with minimal supervision.
Excellent written and verbal communication skills.
Excellent organizational skills.

English_Required,_Bilingual_Preferred
