Hours: Full-time (37.5 hours per week, Monday to Friday)

Job Description: We are currently seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to join the Fenland Adult Locality Team within Cambridgeshire and Peterborough NHS Foundation Trust (CPFT). This role offers an excellent opportunity to support adults with complex mental health needs in a community setting, delivering high-quality care and making a meaningful difference in service users' lives.

The successful candidate will act as a care coordinator within a multidisciplinary team, providing assessment, treatment planning, and support to individuals across the Fenland locality.

Key Responsibilities:
Provide comprehensive mental health assessments and risk evaluations for adults with moderate to severe mental health conditions.

Act as care coordinator for a defined caseload, ensuring care plans are recovery-oriented, person-centred, and regularly reviewed.

Deliver evidence-based interventions and support service users to manage their conditions in the least restrictive setting.

Work collaboratively with psychiatrists, psychologists, social workers, GPs, and third-sector providers to ensure integrated care.

Maintain accurate and timely documentation in line with CPFT clinical governance standards.

Attend MDT meetings, CPA reviews, and contribute to safeguarding procedures as needed.

Requirements:
Registered Mental Health Nurse (RMN) with current NMC registration

Minimum 2 years' post-registration experience in community mental health

Strong assessment, risk management, and care coordination skills

Experience working under the Care Programme Approach (CPA)

Ability to work autonomously and collaboratively within a multidisciplinary team

Full UK driving licence and access to a vehicle (required for community visits)
