Care Coordinator – £14.50 per hour
Our client, a warm and welcoming care service dedicated to providing high-quality, person-centred support, is looking for an experienced Care Coordinator to join their compassionate team. As a Care Coordinator, you’ll play a vital role in ensuring residents receive exceptional care tailored to their individual needs, preferences, and goals. You’ll be the bridge between the care team, health professionals, and the people being supported—helping everything run smoothly and ensuring no detail is missed.
This role is ideal for someone with experience in a senior or supervisory care role who’s looking to step up, take ownership, and make a real difference in people’s lives.
What You’ll Need
- Level 3 in Health and Social Care (or equivalent).
- Experience in a senior care role or supervisory position within a care setting.
- Confident in managing rotas, care plans, and medication administration.
- A kind, organised, and proactive approach to care coordination.
- Strong communication skills and the ability to lead by example.

Key Responsibilities
- Coordinate and oversee care delivery for residents, ensuring it meets person-centred standards.
- Organise and manage staff shifts, ensuring consistent and appropriate cover.
- Administer and accurately record medication.
- Liaise with GPs, nurses, and other healthcare professionals as needed.
- Maintain and update computerised care plans and documentation.
- Support with the induction, training, and supervision of care and support staff.
- Promote a culture of dignity, respect, and empowerment.
