We are currently seeking a dedicated and compassionate Outreach Family Support Worker to provide early help and support to children, young people and their families. This is a pivotal role for an experienced individual who can deliver outreach and home-based interventions with a whole-family approach, particularly at Levels 2 and 3 of need. The role is designed to prevent escalation to statutory services, improve outcomes, and promote access to universal provision.

Key Responsibilities:

  • Hold and manage complex early help cases, undertaking assessments and leading Team Around the Family (TAF) processes.

  • Provide intensive support to vulnerable children and adolescents, including those with emotional and mental health needs.

  • Deliver bespoke packages of support, develop practical parenting strategies, and facilitate family-focused meetings.

  • Use de-escalation methods and solution-focused approaches in line with the Signs of Safety framework.

  • Maintain accurate case records, chronologies and assessments on electronic systems.

  • Undertake home visits and provide ongoing support to families, promoting safeguarding at all times.

  • Support transitions from children's social care and deliver supervised contact where required.

  • Attend regular supervision and contribute to evaluations, planning meetings and service developments.

  • Uphold equal opportunities and anti-discriminatory practice in all aspects of service delivery.

Requirements:

  • Experience working with children, young people, and families in a social care or early help context.

  • Proficiency in completing early help assessments and leading multi-agency plans.

  • Excellent communication, record keeping and interpersonal skills.

  • A sound understanding of safeguarding and child development.

  • Ability to work flexibly across a citywide remit, including home-based interventions.

  • Familiarity with models such as Signs of Safety and Family Outcomes Star is desirable.
