Case Manager - Outpatient Services

Job Description

QUALIFICATIONS: Requires knowledge of person-centered / strength-based counseling, case coordination, and recovery planning using ASAM criteria. Also requires good communication, writing, and organizational skills and a working knowledge of Microsoft Office. Must have Addictions Counselor certification, a related professional license, or be eligible to obtain either within the required time frame. Minimum of 12 months of continuous sobriety required for persons recovering from a substance use disorder. Submit to DHS Schedule L background checks and meet the eligibility requirements for employment. Experience in substance use disorder treatment, degree in a Human Services-related field, LCPC or LCSW, and Bilingual preferred.


  1. Provide person-centered support, recovery coaching, and care coordination while regularly carrying a caseload.

  2. Provide person-centered counseling and education services.

  3. Complete all documentation in a timely manner

  4. Meetings, In-Services, Conferences and Committees

  5. Other duties as assigned
