Position: Care Assistant - Rapid Response

Location: Market Deeping, Lincolnshire

Hours: Various contracted hours (21,28 or 35 hours) over a 24-hour shift pattern

Salary: £21,617 per annum FTE (Fixed Event Contract with potential for extension or permanent role)

About the Role

As a Rapid Response Care Assist, you will offer a 24-hour call-out service, offering support and assurance to customers across the county. This charity also delivers hospital avoidance services, supporting patients' recovery in their own homes through short-term packages of care. You will provide a flexible approach to care and support, working with our customers and their loved ones at a pace that is right for them.

Successful candidates will be responsible for the direct support of our customers, ensuring the care and support they provide is of the highest quality and is tailored to their needs and wants. You'll be responsible for creating care plans, providing assurance, and offering support to individuals who may have fallen and need assistance.

Key responsibilities of the Rapid Response Care Assist include:

Meet the Customer at their home in a timely manner.
On entering the Customer's home, ensure safe access and all facilities are switched on and working.
Ensure the internal layout of the home is safe to support safe mobility.
Establish the Customer's outcomes and agree on an appropriate care plan to achieve them.
Provide comprehensive care and support packages including personal care if required.
To complete short-term care profile and other associated paperwork in partnership with the Customer.
To communicate with family, friends, and neighbours if the Customer wishes you to do so.
Respond to a Telecare alert by visiting the Customer's home.
Take appropriate action of the above to remedy the issue/concern in conjunction with the Wellbeing Service and/or other agencies as necessary.

About You

The charity is looking for a passionate care assistant that is looking for a more varied and responsible position.

To succeed in the role of Care Assistant - Rapid Response, you will be:

Willing to work a variety of shifts including weekends and night shifts
Understanding and appreciation of the needs of older people and people at risk
Positive attitude and a good sense of humour
Self-motivated and able to work alone
Satisfactory Enhanced DBS check
Full driving licence with access to own vehicle

About the Organisation

The employer is an independent local charity, working to support the older people of the Lincoln & South Lincolnshire. They have been working in the local community to help older people for over 50 years in various forms and are a brand partner to Age UK National.
