Social Worker - MH Team - Sheffield Council - £30ph

Based at Michael Carlisle Centre and across SHSC Trust work sites. The candidate will need to be an experienced social worker with a mental health background. Core tasks and duties as follows:

• Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
• Some mental capacity assessments, BI decisions and safeguarding as required
• Attending 117 meetings, ward rounds, professional meetings
• Facilitating learning opportunities for ASYEs through shadowing
• Participation in duty rota
