Key Responsibilities:

Mental Health Social worker to complete Care Act Assessments, Reviews, Safe guarding, care coordination under CPA Framework.

Manage a caseload of 25 clients and be part of a Duty Rota, which may include screening, triaging and new referrals.

To be part of an integrated Mental Health and wellness Team.

Essential Requirements:

Degree/Masters in Social Work and SWE registration.

*

2+ years of post-qualification experience.

*

Strong knowledge of UK social care legislation and practice.

*

Right to work in UK
