Your Role

As a key member of the Hospital Team, your responsibilities will include:

  • Hospital Discharge Planning: Complete Care Act assessments and arrange safe, timely discharges through the Discharge to Assess (D2A) model.

  • Integrated Working: Collaborate with health professionals, therapists, housing officers, and others to coordinate discharge planning and aftercare.

  • Reablement Focus: Promote independence and recovery, aiming to minimise reliance on long-term care and avoid hospital readmission.

  • Safeguarding Adults: Respond to concerns in accordance with Section 42 of the Care Act and local safeguarding policies.

  • Mental Capacity & Advocacy: Conduct Mental Capacity Assessments and Best Interest decisions; ensure access to advocacy when needed.

  • Care Reviews: Monitor and evaluate care packages for effectiveness, quality, and value for money.

  • Documentation: Maintain accurate records in line with legal and local authority standards using electronic case management systems.
