POSITION: Community Habilitation/Respite Provider - Full Time

Community Habilitation Services: Community Habilitation is a service delivered between one staff member and one individual receiving the service. Direct Care Providers work on goal areas that the individual has identified they'd like to gain more independence in. The hope is to work toward the service recipient's long term life goals such as living independently some day or maintaining that ability. With that said, this service is appropriate for any level of need because the focus is to gain and maintain as much independence as is possible for each unique person. This service is usually delivered in the combination of community settings and in the individual's home.
DUTIES AND RESPONSIBILITIES: The Habilitation/Respite Provider is responsible for providing direct care through implementation of Habilitation Plans. Habilitation/Respite Provider Half-Time's specific duties and responsibilities include, but are not limited to the subsequent items and are characterized in the following five standards of performance, defined as below:
JOB KNOWLEDGE: • Provide supervision and all direct care required to maintain safety and comfort of the consumer in the absence of family members • Transport or accompany individuals on trips to community outings, appointments or recreation activities for the purpose of training in mobility, community orientation and awareness • Assist individuals and/or families with, or if necessary, completing daily or routine domestic or personal care tasks • Foster and promote skills leading to independent living • Assist with the development of Habilitation Plans and following these plans for training and assistance • Provide training and assist consumers to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required. • Act as role model to consumers • Provide supervision to consumers during program hours and during community outings as to ensure their health and safety. • Any additional duties as assigned
TEAMWORK: • Encourage and maintain a positive working relationship with all departments in the agency, parents, staff, residents and community partners • Communicate all health, safety, consumer and/or program concerns, sensitive situations and incidents to supervisor and family.
DOCUMENTATION: • Complete required documentation in conformance with rules, regulations, policies and procedures and billing practices
COMPLIANCE & ACCOUNTABILITY: • Identify and report any untoward incidents • Identifying, reporting and/or correcting all safety hazards including physical plant problems and/or problem work practices and assist with investigating all work-related accidents as requested • Participate in any investigations as required • Comply with all State and Federal regulations along with agency policies and practices
TRAINING & DEVELOPMENT: • Obtain and maintain required training certifications that includes CPR, First Aid and SCIP • Attend all required training, staff development programs and staff/agency meetings as required by the agency and/or State and Federal regulations • Attend requested agency approved training in a related topic that is above and beyond the required agency training
SPECIAL REQUIREMENTS: Possess good interpersonal, written, oral, and communication abilities and effective time management/organizational skills
Must be able to work scheduled hours that may include weekend and/or holiday timeframes and maintain an acceptable work attendance as defined in the agency's personnel policy manual and handbook.
Be computer literate with knowledge in agency operating systems and applicable software required for department needs. Have regular access to a computer with internet capability
Demonstrate sound judgment in normal work and emergency situations
Must physically be able to perform required certifications and lift 50 pounds Possess a valid driver's license acceptable to the agency's insurance carrier
QUALIFICATIONS: High School Graduate with some knowledge of or experience working with the mentally retarded/developmentally disabled preferred.
A combination of education and experience that in the opinion of the reviewing agency, exceeds the above standards of the program
