Department: Counseling and Case Management

Essential Functions:

Role Description

45%

Supervision, Consultation & Training

1. Supervise graduate interns and staff as assigned and complete performance evaluations as indicated.

2. Manage HMI's membership intake process to ensure a safe and seamless transition for new members into programs and services.

3. Collaborate with counseling team to ensure comprehensive review of all new intakes.

4. Provide trainings to counseling staff and interns on procedure and best practices for conducting intakes and mental health screenings.

5. Assist in the collection and reporting of data and other pertinent program information required by agency funding sources and program evaluators.

6. In collaboration with the Director of Counselling & Case Management, work with the Data, Program Administration, & Grants Management teams around counseling model treatment plan & quality initiatives.

7. Support Director of Counseling and Case Management Services in the management and oversight of funding sources; grant acquisition, implementation and evaluation.

Clinical Case Work Responsibilities

1. Provide necessary referrals and follow-up for HMI members to access additional services.

2. Facilitate supported referral services with external providers to HMI services.

3. Collaborate with program staff and partner organizations toward the comprehensive care of youth members.

4. Conduct safety assessments (suicidal/homicidal ideation, housing, legal, substance use, and abuse/neglect) as indicated.

5. Provide referrals for both internal and external resources/providers as needed.

6. Assist in the day to day on-call services/operations within the Mental Health Counseling & Case Management track.

7. Facilitate weekly counseling group for trans, non-binary and gender non-conforming youth.

8. Provide individual-level clinical support to youth members, including:

a. Weekly supportive counseling for a small caseload of 5 youth members and their families

b. Creating youth re-entry plans related to behavior change/disciplinary action in collaboration with the Director of Counseling and Case Management

c. On-call counseling

d. Crisis intervention

e. Escort youth to Pantry and conduct needs assessment for potential referrals to services.

9. Conduct assessment, problem definition, treatment planning and case coordination of youth to support stabilization and participation in HMI programs and in school with anti-oppressive methodologies.

Administrative Duties

1. Create materials, processes and procedures for intakes, including brochures, counseling group calendar, and intake station.

2. Support Member Navigator in creating member identification cards and critical data entry into Salesforce (agency database) to enhance coordination of care.

3. Act as the liaison with communications team to ensure updates are made as needed for website and social media around intakes, outreach and groups.

4. Participate in Youth Services Departmental Management Team Meeting (weekly).

5. Participate in graduate internship planning meetings.

6. Develop program designs as determined by clinical trends, community needs, and contractual requirements.

7. Support in monitoring and meeting all contractual requirements within Counseling and Case Management department to meet compliance standards.

8. Attend regular trainings on mental health interventions and supervision.

9. Co-research and co-write academic papers & studies in collaboration with the Counseling team as needed.

10. Manage the implementation of targeted evaluation tools to evaluate success of programmatic interventions in achieving outcomes and overall agency mission.

11. Complete electronic data reporting immediately following service provision into Salesforce, including individual counseling progress notes, group notes, referral documentation, and other service documentation as directed.

12. Meet and exceed all compliance standards for HMI and contracts by meeting deadlines, meeting outcomes and maintaining best practices in the delivery of service.

Knowledge, Skills and Abilities Required:

  1. LCSW, LMSW or LCAT Licensure required.
  2. At least three years experience with diverse population of adolescents in an urban setting, including knowledge of lesbian, gay, bisexual transgender and questioning youth from various cultures and backgrounds.
  3. SIFI certification and three plus years supervisory experience with graduate interns preferred.
  4. Ability to exercise independent judgment when appropriate; strong decision-making skills.
  5. Strong oral and written communication skills.
  6. Experience facilitating group discussions and managing a variety of small and large group dynamics.
  7. Excellent verbal and written communication skills, including conflict mediation and public speaking.
  8. Knowledge of harm reduction practices, trauma-informed care, and positive youth development models.
  9. Knowledge of and commitment to using a liberation-focused and anti-oppressive lens in their work.

Computer Skills: Strong with proficiency in Zoom, Doxy, Microsoft Word, Excel, PowerPoint and Outlook.
