About the Role Hammersmith & Fulham has expanded its Transitions Team, and we're excited to offer an opportunity for motivated and qualified Social Workers to join us. You will play a pivotal role in supporting young people transitioning from Children's Services to Adult Services. This position requires a deep understanding of Safeguarding, creative community-based planning, and effective communication with young people and their families.
Key Responsibilities:
- Manage a caseload of young people transitioning to adulthood, ensuring smooth communication and planning.
- Conduct eligibility and outcome-based care assessments in line with the Care Act 2014.
- Liaise with professionals across Health, Education, Police, and other agencies to provide comprehensive support.
- Maintain accurate records and input data into our core system.
- Participate in team duties, reflective practice meetings, and local Social Work conferences.
- Stay updated on legislation, policies, and resources relevant to our services.
About You Knowledge & Skills:
- Qualifications: Diploma/Degree in Social Work or equivalent, registered with Social Work England.
- Experience: Significant multi-agency experience, with a focus on Special Educational Needs and Disabilities (SEND), Health (including CHC funding), Care Leavers, Housing, and Direct Payments.
- Legislation: Strong understanding of the Mental Capacity Act, Care Act, Children and Families Act, and safeguarding procedures.
- Assessment: Proficient in conducting outcome-based assessments and implementing care plans.
- Teamwork: Excellent ability to work collaboratively with other professionals and agencies.
- Communication: Skilled in maintaining appropriate records and effectively communicating with service users, families, and professionals.
Corporate Requirements Civil and Other Emergencies: Support the Council's emergency planning and response efforts, participating in training and testing as required.
Digital: Utilize Council IT services for efficient and secure information management.
