Job Description:
Liquid Personnel is recruiting a skilled and proactive Social Worker on behalf of its client in Tower Hamlets. Based within the Locality Team, you will support adults with care and support needs across a defined area. The role focuses on promoting independence and delivering timely, strengths-based interventions through a person-centred approach.
What will your responsibilities be?
- Undertake Care Act assessments, reviews, and safeguarding enquiries.
- Develop and implement care and support plans in partnership with individuals and carers.
- Work collaboratively with health, housing, and community partners to deliver integrated care.
- Respond to crisis situations and manage risk effectively.
- Promote early intervention and preventative strategies.
- Maintain accurate and timely case records using appropriate systems.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
